This workshop will highlight the DESE Transition Assessment Framework. Participants will learn the Six Phases of Transition Assessment.
.Guiding Questions- these are the questions you will ask yourself about what you already know about the student and what you still need to learn.
.Assessment Plan-developing a plan that takes into consideration the critical response characteristics and needs of the student.
.Assessment Selection in this phase, you will focus on the skills you need to assess and the methods/strategies for assessing these skills.
.Using Data-relates to how you and the students will actually use the data and communicate with family and others.
.Measurable Post-Secondary Goals are based on transition assessments. They help students define/refine career and life goals.
.Annual Goal-what skills and knowledge does the student need to meet their goals as identified by state standards and help the student progress toward achievement of their postsecondary goals?
